Learning Environment Form

If you would like to request a specific learning environment for your child for the upcoming school year, please complete this form and email to leslie.jewkes@canyonsdistrict.org. Requests will be accepted through April 30. These requests will be considered in addition to teacher input and components to create balanced classes.
